Primary and social care services during Covid-19
As the current covid-19 situation is still on-going, your local Clinical Commissioning Groups and Healthwatch would like to understand how this may be affecting you. There have been many temporary changes to the way health care is being delivered in a short space of time to delay the spread of the virus.

We want to improve the care we are providing at this very difficult time to ease daily pressure and improve your health and wellbeing. Your feedback will also be used to help us develop the way health services are offered following the Covid-19 pandemic. Your experiences of health and social care services during this unique time, good and bad, are incredibly valuable to us.

This survey will ask you about the following topics:
Information and advice
Using your GP practice
Using your pharmacy
Social care support
Ongoing healthcare
Mental health and wellbeing
